Patient-intake, registrar for out-patient procedures, Update and verify all demographic  information by data entry, driver license, insurance cards, verify authorization (hard copy) by downloading from insurance Web sites, collect deductibles, co-pay’s, cash /credit card transactions. Maintain financial daily log, balancing. Answer heavy phone lines, scheduled hundreds of appointments daily for Physical Therapy, (PT) Occupational Therapy, (OT) and Speech, (SP) therapy for adults and pediatrics, initial consultations, re-evals, and continuation treatment. As a case manager assistant, I worked closely with the therapist, patients and their families; I networked with the physician’s office and insurance companies to get authorization for evaluation’s and continuation of care. Complete file review with accurately, provide (hard copy) authorization and paperwork and file documents in file, enter daily notes manually and in database, once authorized, forward file to the scheduling department for scheduling. I maintained a heavy case load at the same time making sure patients and their families were informed of future treatment.

I provided patient information status to Lawyer’s and Insurance companies, in a medical Report form. I scheduled all of outer patient referral’s services x-Ray, Labs, MRI, and CT scan.
